Employees and spouses/domestic partners enrolled in an SEH health plan 11/1/23-07/31/24 will work with their physician to complete their annual physical.
Your annual physical must occur between 11/1/23-10/31/24 while enrolled on an SEH medical plan. The completed form must be submitted in ADP by 11/15/24.
Consistent with previous years, to receive the $64.45 monthly Wellness Credit, both the employee and their spouse/domestic partner on the SEH health plan must complete their annual physical and complete the required steps.
Eligibility
- Employees and/or spouses/domestic partners on the health plan 11/1/23-07/31/24 must participate in order to receive the wellness credit in 2025.
- Those enrolled on the health plan between 11/1/23 and 7/31/24 must work with their physician to complete the form by 10/31/24 and submit the required form in ADP by 11/15/24 to receive the wellness credit in 2025 completed during this timeframe.
- For those adding SEH medical 11/1/23-7/31/24, an annual exam completed in the same year will be accepted if the form is completed.
- Those enrolled on the health plan after 07/31/24 will automatically be eligible for the wellness credit in 2025.

What if I am a smoker?
To obtain the wellness credit, you must participate in a tobacco cessation program, but quit status at the end of the cessation program is not a requirement. The Tobacco Cessation section details current cessation options.
Tobacco Attestation Requirement
As part of the Physician Engagement Program, participants are required to complete the Tobacco Attestation during open enrollment and certify on the required form. If you are a tobacco user, voluntary completion of the Tobacco Attestation is required to obtain the wellness credit in 2025. Please note, the Physician Engagement Program is a voluntary program.
Reasonable Alternative Standard for Tobacco Users
Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees. If you are a tobacco user, we offer a tobacco cessation program through Healthpartners. The program is no cost to you. If you complete the tobacco cessation program by 6/30/24 you will qualify for the wellness credit in 2025.
Following completion of the cessation program, employees and spouses must provide a program completion certificate to HR1@sehinc.com to initiate their wellness credit. Participants still may be tobacco users at the end of the cessation program.
